Please make sure the following steps have been completed:
- Complete Section I. Sign and date.
- Attach enrollment confirmation from your concurrent institution.
- Obtain host institution official's signature.
- Submit in person or by fax to the Registrar's Office located in the Bell Building on the 2nd floor.
The Registrar's Office will forward the form to the Office of Student Financial Assistance for processing.
